Do you have the most recent version of the USB loader? Have you installed all the IOSs needed for the USB loader? Are you booting directly from HBC or are you using a forwarder channel?
Did you download it from this site? (If not, you should as the one posted here are tested and updated regularly) Have you tried both USB slots? (I know USB Loader GX will only work with one of the USB slots. On mine it's the bottom one if my Wii is laying down.).
